Changhua County Hepatitis C Eradication Program: Collaborative Care Across Departments, Projects, Levels, and Disciplines.

The Changhua County Hepatitis C Eradication Program was launched in 2019. The program adopts a collaborative care strategy across departments, projects, levels, and disciplines, embedding the Hepatitis C Eradication Program into existing health care plans. This reduces the duplication of work processes among different service providers targeting the same population, while also increasing the efficiency and effectiveness of the program. As of 2022, the treatment rate of the entire Changhua County population has reached the WHO's eradication goal. The collaborative care integration strategy implemented in this program can be extended to other health promotion projects.

Result

Type B and type C is the viral hepatitis with the largest number of domestic infections. It causes about 13,000 people each year. Liver cancer is the second place for cancer death. In view of this, Changhua County launched the Aliexing Plan of Hepatitis C in 2019, adopting a common care strategy of cross -department, cross -plan, cross -level, and inter -section. Different service providers' repeated workflows on the same target group also improved the implementation efficiency and effect of the implementation of liver C and except the plan. The treatment rate of the entire county's population in 2022 has reached the target of WHO. It adopted the same care integration strategy to extend to other health promotion plans.
